A state name does not enable identical actions for every record. Completed treatment plans and completed paper prescriptions have different edit rules. Use the record's current actions and specific guide.

| Clinic connection | Access relationship, not household membership or debt settlement |
| Household | Owner sharing group; primary and member actions differ |
| Active / deactivated / deceased | Clinic activity/visibility differs from life status |
| Draft | Prepared record, not proof of administration, signature or collection |
| SOAP / examination | Subjective, Objective, Assessment, Plan workspace; sales link separately |
| Signature | Clinical closure/identity trail, separate from preview or owner consent |
| Revision / pinned result | Current editable content versus signature-time preserved content |
| Protocol / program | Vaccine steps/transitions versus a program of published protocols |
| Scheduled / administered vaccine | Intended date versus actual clinical administration |
